Issued to:
- Persons whose identity documents are lost, stolen or damaged.
- Persons who applied for the change of personal particulars. (The necessary supporting documents to justify an application for the change of personal particulars in the Population Register should be attached.)
- Persons whose citizenship status has changed.
- A woman who entered into matrimony or would like to resume any of her previous surnames.
- A person who is temporarily residing abroad and who has previously been in possession of an identity document, and
- An immigrant if the validity of his permit
for permanent residence has not yet expired.
An application form BI-10 is required for the reissue of an identity document. However, in cases where an applicant's fingerprints have not yet been submitted/recorded in the Population Register, an application form BI-9 on which fingerprints are taken on the reverse side thereof, should simultaneously be submitted. The thumb prints required on the form can only be taken by an official of the Department at a regional or district office.
A fee of R13-00 is payable for every reissue and the applicant has to furnish two recent identity document photographs.
PLEASE NOTE: Application fees for re-issues of IDs have been waived until the closure of voting registration for 2004 elections.
If a person is married, the marriage certificate must be submitted.
Should a woman apply for the reissue of an identity document and wants to resume any of the surnames she was previously known by, documentary proof must be submitted to prove that she is entitled to that surname.
Photographs
Photographs must comply to the following specifications:
The photographs must be: - two identical colour photos for passports
- two identical colour or black and white photos for IDs
- not more than one (1) month old
- 35mm in width and 45mm in height
- a close-up of your head and top of your shoulders so that your face takes up 70 – 80% of the photograph
- of high quality with no ink marks or creases and not damaged
- show you looking directly at the camera
- have appropriate brightness and contrast
- be printed on high-quality photo paper
- show your natural colour
- show your eyes open and clearly visible - no hair across your face
- show you facing square onto the camera, not looking over one shoulder (portrait style) or tilted
- show both edges of your face clearly
- be taken with a light grey or plain cream-coloured background
- be taken with uniform lighting and not show shadows or flash reflections on your face and no red-eye
- show you alone (no chair backs, toys or other people visible)
The Director-General may reject the copies of the photographs if they do not, in his or her judgment, comply with the requirements mentioned above, or if he or she deem them unsuitable in other respects, in which case the person concerned shall provide two new copies at his or her own cost.
Identity document and passport photographs could be taken at regional or district offices equipped for this service, at a cost of R13-00 for a frame of four black and white photographs.
